*Prior to your first visit, if you wish your Provider to direct bill for you, please enter your insurance company’s name and your policy numbers on your online health history intake form.
Cancellation Policy
Please note that cancellations within 24 hours of your appointment are subject to a cancellation fee of the full dollar value of your scheduled treatment.
